Fonasa, Isapres and private insurance: different roles
Fonasa is Chile's public health insurer. Isapres are private statutory health insurers. For workers required to contribute, the legal reference is 7% of assessable earnings, subject to the applicable ceiling. Ask your employer how this applies to your contract and whether any special social security arrangement applies.
Private supplemental insurance is an additional contract. It may reimburse eligible expenses after statutory benefits. It does not automatically replace enrollment in Fonasa or an Isapre. If you hold international or travel insurance, check its scope and compatibility with your employment situation in Chile.

Documents and effective dates to confirm
- Ask what identification and proof of income the institution requires for your situation. The regulator's guidance mentions a passport to verify a foreign applicant's signature and documents such as an employment contract or payslip to establish income.
- Confirm how to register if your Chilean RUN or identity card is pending. A passport alone does not necessarily complete every online enrollment process.
- If family members are joining you, check each person's requirements and request the total household price.
- Obtain written confirmation of acceptance, any restrictions and the start of benefits. Getting a quote, signing and having active coverage may happen on different dates.
When Fonasa may be a suitable option
Fonasa may suit you if its public network and cost fit your priorities. Copago Cero provides free care under the institutional care arrangement, known as MAI. It does not make all private care, private hospital wards or services under the free-choice arrangement (MLE) free.
For private care, check the applicable arrangement, the provider's agreement and your copayment. Ask about available options for specific services or planned surgery before choosing a hospital. Enrollment alone does not establish the cost or waiting time of a particular service.

When an Isapre plan could be more suitable
A plan may be more suitable if its contractual benefits for your chosen hospital and services provide protection you value and can afford. Check inpatient care, outpatient care, emergency care, fee schedules and limits separately. The insurer's name or a headline percentage is not enough.
New enrollment may require a health declaration and the institution's assessment. Confirm acceptance and the full price, including GES and other applicable components. A lower copayment for one service does not establish that a system is better in every scenario.
For high-cost care, check whether your contract includes CAEC, its deductible and designated network. It requires activation and specific conditions; it does not provide free care at any private hospital.

Fonasa plus supplemental insurance: what to check
This can be a useful combination if the policy accepts your Fonasa status and covers the services and network you need. Compare your statutory contribution, the insurance premium and the costs you would still pay.
- Inpatient care: check room charges, intensive care, professional fees, medication and supplies, as well as annual or per-event limits.
- Emergency care: check the consultation, tests and any resulting hospitalization separately. A headline reimbursement rate may not apply to the whole bill.
- Prior benefits: ask what happens if Fonasa does not reimburse a service. Special policy rules may apply.
- Terms: check deductibles, waiting periods, exclusions, network, age limits and renewal. For employer-provided cover, ask what happens when you change jobs.
- Payment: establish whether there is direct settlement or you must pay first and claim reimbursement later.

Life-threatening emergencies and subsequent private care
A life-threatening emergency requires immediate care and medical certification. Do not delay seeking care to compare insurance. Chile's Emergency Law protects access but does not make all private care free.
If you have Fonasa and choose to remain at a private hospital after stabilization, subsequent care may fall under MLE. Confirm transfer arrangements and applicable funding with the institution. Supplemental insurance pays only according to the policy terms.

A checklist before choosing
- Write down your employment situation, budget, household members and city of residence.
- Choose the hospital and location to assess. Request the terms for a consultation, planned hospitalization and an emergency.
- Compare total annual cost and your potential expenses in those scenarios using documents for both alternatives.
- Confirm acceptance and effective dates before ending existing cover. Ask for clarification if any condition is unclear.
Planea tu Salud lets you explore Isapre plans and contracts without registration. The comparison tool does not quote supplemental insurance or calculate Fonasa benefits. Those documents require a separate review.
